Pillars Community Health takes some private insurance as well as Medicaid and self-pay. Some services are offered free of charge; others are offered on a sliding fee scale (payment based on income). We do not turn clients and patients away if they cannot afford to pay. If you have questions about what you may owe or want to discuss your ability to pay, please call 708-PILLARS (708-745-5277).
If you are uninsured, our team of benefit specialists can work with you to see if you are eligible for any available insurance coverage and assist you with other benefits, such as SNAP enrollment.
Our Sliding Fee Scale Program for the Health Center

We also offer a sliding fee scale program to those who are uninsured or underinsured and low income. Even if you have insurance, we may be able to apply the sliding scale to your copay and deductible. Any payment that is required for your visit is due at the time of your appointment.
Qualified patients are charged on a sliding fee scale, which is based on family size and household income. In order to qualify for a discount your family income must be at or below the amount shown in the table.
In order to qualify, patients must provide proof of identification, proof of family size and proof of income. This information must be updated every 12 months.
- Identification may include State Driver’s License or ID Card, Matricular Consular de Alta Seguridad (Identification card issued by the Government of Mexico), Passport, or similar identification from other countries of origin.
- Family Size includes any dependent children up to 18, spouse or partner, children up to age 26 who are attending college and any individuals over the age of 18 for whom the applicant has legal responsibility to support that are living in the home. Court documents and school transcripts may be required for adults over the age of 18 who are not the main applicant.
- Proof of Income should include at least 4 current paystubs for each family member AND your most recent tax return and any tax returns of those living with you who are not included in your tax return. If you cannot provide these documents, please call our enrollment staff (708-352-0081 + option 3 for Benefits) for get further instructions on other ways to meet this requirement.
