All patients are welcome at Community Nurse regardless of insurance status. Patients without the ability to pay will not be denied services.

We accept a wide variety of commercial, Medicaid and Medicare managed care insurance plans. The list below may change so please contact our Benefits and Enrollment Counselors at 708-352-0081 for any additional plans. If you are uninsured, our team of benefit specialists can work with you to see if you are eligible for any available insurance coverage and assist you with other benefits, such as SNAP enrollment.


  • Blue Cross Blue Shield Medical – All Plans are accepted In Network including Exchange Plans
  • All other commercial insurance is accepted as Out of Network
  • You may qualify for additional sliding scale fee discounts (on deductibles or copayments) based on your income. Contact a benefits counselor for more information.


  • We accept Medicare Part A – Even for your primary care.

Advantages of Using Medicare at Community Nurse:

  • When using your Part A insurance at a Federally Qualified Health Center, you do not need to meet your deductible before receiving benefits.

We also offer a sliding fee scale program to those who are uninsured or underinsured and low income. Even if you have insurance, we may be able to apply the sliding scale to your copay and deductible. Any payment that is required for your visit is due at the time of your appointment.
Qualified patients are charged on a sliding fee scale, which is based on family size and household income. In order to qualify for a discount your family income must be at or below the amount shown in the table.

In order to qualify, patients must provide proof of identification, proof of family size and proof of income. This information must be updated every 12 months.

  • Identification may include State Driver’s License or ID Card, Matricular Consular de Alta Seguridad (Identification card issued by the Government of Mexico), Passport, or similar identification from other countries of origin.
  • Family Size includes any dependent children up to 18, spouse or partner, children up to age 26 who are attending college and any individuals over the age of 18 for whom the applicant has legal responsibility to support that are living in the home. Court documents and school transcripts may be required for adults over the age of 18 who are not the main applicant.
  • Proof of Income should include at least 4 current paystubs for each family member AND your most recent tax return and any tax returns of those living with you who are not included in your tax return. If you cannot provide these documents, please call our enrollment staff (708-352-0081 + option 3 for Benefits) for get further instructions on other ways to meet this requirement.
