Community Nurse Health Center partners with other health care providers and social service and other organizations to provide comprehensive services to our patients, and to reach out into the communities we serve to assure more people have access to quality services. In addition to the partners noted below, Community Nurse works closely with area schools, churches and faith partners, food pantries, and businesses to increase access to care.

The Community Healthcare Network
Community Nurse leads The Community Healthcare Network of the Western Suburbs, a collaboration of providers that delivers coordinated health services. Services include low cost comprehensive primary care, specialty physician care and diagnostics, prescription medications, mental health services and hospitalization.

Eligibility: Uninsured Adults, ages 19-64, whose family income does not exceed 200% of the Federal Poverty level. Patients must reside in one of the following communities: Brookfield, Broadview, Burr Ridge and Hinsdale (Cook County only), Countryside, Hodgkins, Indian Head Park, Justice, La Grange, LaGrange Highlands, LaGrange Park, Lyons, McCook, Summit, Westchester, Willow Springs and Western Springs and the Lyons Township areas of Bridgeview and Hickory Hills.
Mental Health Services Partner
Community Nurse Health Center and Pillars Community Services work together to make mental health services and counseling available to patients at Community Nurse, and to integrate physical and behavioral health medicine to improve and coordinate care among providers.

Health Care for the Homeless Network
Community Nurse works closely with area organizations to increase access to health care services for persons experiencing homelessness in our target communities, including women and children who have experienced domestic violence who are living in sheltered houseing. A Registered Nurse from Community Nurse provides triage services at the BEDS Day Program location (Open Monday – Friday, 10am – 2pm at Emmanuel Episcopal Church, 203 Kensington, La Grange – enter at Catherine Avenue, wooden door marked “BEDS Plus”. Center is on the 2nd floor.) and our staff works closely with BEDS to facilitate access to medical, mental and oral healthcare services at our clinic sites on Calendar Avenue.
