Who We Are

Service Area
Since 1921, Community Nurse Health Association has been providing health care and social services to low income, working families in the area. We are a primary care health and dental facility, our patients’ medical “home,” where families develop a consistent relationship with our staff of dedicated doctors and nurses.

Our service area encompasses the following 15 suburbs:
