If you are a Community Nurse patient, you can enroll in our online patient portal which will allow you to make appointments, ask questions to our staff and providers, view your test results and medical history, and access educational material. To access our Online Patient Portal, click here.
Community Nurse health care services are available to all families and individuals regardless of your insurance status or ability to pay. A modest Sliding Fee Scale (fees listed are effective April 1, 2017) is available for low-income and uninsured individuals. Medicaid is accepted. All pregnant women are eligible for All-Kids Moms and Babies to cover costs of prenatal and postpartum care, including exams, care management, health education and assistance with benefits eligibility. We also have bi-lingual English –Spanish staff to assist you.
While you are welcome to access our services regardless of your place of residence or income status, some of our low-cost services and referral services are available only to residents with certain income levels and residents (below 200% the federal poverty level) of the following communities:
Broadview, Brookfield, Burr Ridge (Cook County), Countryside, Hinsdale (Cook County), Hodgkins, Justice, Indian Head Park, LaGrange, LaGrange Highlands, LaGrange Park, Lyons, McCook, Summit, Westchester, Willow Springs and Western Springs and the Lyons Township areas of Bridgeview and Hickory Hills. Some Behavioral Health counseling services provided at Community Nurse are provided by staff of Pillars Community Services. Pillars is a service partner of Community Nurse Health Center, but a separate health services provider. Pillars staff will be clearly identified to all patients. Pillars applies its own schedule of fees and collects payment for the services provided by its staff to patients at Community Nurse.
Some Nutritional counseling and education services at Community Nurse are provided by faculty and graduate level students of Dominican University. Dominican University is a service partner of Community Nurse Health Center, but a separate entity. Dominican University staff will be clearly identified to all patients. You will not receive a bill for services rendered by Dominican University.
