Community Nurse leads The Community Healthcare Network of the Western Suburbs, a collaborative of providers that deliver coordinated health services. Services include free and low cost comprehensive primary care, specialty physician care and diagnostics, prescription medications, mental health services and hospitalization. Network partners include:
• Adventist La Grange Memorial Hospital
• ALMH Family Medicine Center
• Pillars Community Services
• Access DuPage
• More than 75 local primary and specialty care volunteer physicians
Patients co-payments for services and prescription medications do apply.
Prescription medications copays:
$10 generic
$20 brand name
$50 nonformulary
Eligibility: Uninsured Adults, ages 19-64, whose family income does not exceed 200% of the Federal Poverty level. Patients must reside in the following communities:
Brookfield, Broadview, Burr Ridge and Hinsdale ( Cook County only), Countryside, , Hodgkins, Indian Head Park, Justice, LaGrange, LaGrange Highlands, LaGrange Park, Lyons, McCook, Summit, Westchester, Willow Springs and Western Springs and the Lyons Township areas of Bridgeview and Hickory Hills.
