Criteria:
1. Children 0-18 with Medicaid (AllKids) and uninsure adults 19-64 years old.
2. Reside within the 14 communities that make up our service area for 30 days or longer.
3. Income at or below 200% of the Federal Poverty Line. Family size of 1=$21,780 per year, for each additional dependent under 19 add $7,640.
4. If you qualify for our services, limited co-pays apply and services are based on availability.
